The state of Texas requires all teachers to be trained on the Professional Development and Appraisal System (PDAS), which is the state's approved instrument for appraising teachers. If you have documentation which you can provide to your principal demonstrating that you have completed PDAS training in another Texas district within the last three years, you can sign a waiver form. If you have not attended PDAS training in another Texas district, you will be required at your new hire meeting to sign up for PDAS training.

W - 4 - Complete the lower portion of the form with the total number of allowances you are claiming. Do not cut the form.
Social Security Form - Employee ID# is the social security number.
Substitute Resignation Form - Complete if you are an active substitute for Plano ISD.
