    1. If you have signed in at Carlisle before, fill out the form and we will try to have your badge pre-printed for you.
    2. If you have NOT signed in at Carlisle THIS SCHOOL YEAR, please bring your driver's license by the Carlisle office to have it scanned in as we can't pre-print a badge until your driver's license is scanned.

    **Please bring your driver’s license and arrive 20 minutes prior to your child/grandchild’s scheduled lunch time.**
