I understand I have received the Employee Child Care Center (ECCC) Handbook.
Within the handbook, I have received the foregoing documents and understand that is my right and responsibility to read the provisions outlined within each document and to present any questions or comments I may have to the PISD Employee Child Care Center Administration:

Required Forms
I understand the following forms are required before care can begin at the ECCC:
- Child Assessment Form (2 pages)
- Emergency Medical Authorization
- Admission Requirement PISD Employee Child Care Center Health Statement
- Operational Policy on Infant Safe Sleep
- Operational Discipline and Guidance Policy (2 pages)
- Licensing and Insurance Information & Acknowledgement of Receipt
- Signature of Acceptance for Online Enrollment/ Admission

Requirements for Attendance
The immunization requirements for my child will be met before the first day of care attendance.
I understand that my child must be independently potty-trained in order to attend the 3 & 4 year old classrooms at the Employee Child Care Centers.

Items from Home
I will provide a minimum of 5 sets of labeled clothing (underwear, pants, shirts, and socks) and one set of shoes to be kept at the center in my child’s cubby.
I understand it is the parents' responsibility to provide lunch, as lunch is not provided by the centers. Please keep in mind, the centers are a nut free zone when preparing and sending lunches from home. Healthy, nutritionally-balanced lunches are encouraged. AM/PM snacks and milk during lunch time are provided by the centers.

Behavior Concerns
Any concerns regarding behavior management with my child at the center will be discussed with appropriate center staff and parent/s. If a child has persistent behavior problems that interfere with the general welfare of others, they may be temporarily or permanently removed from the program.

Schedule
The schedule for the Employee Child Care Centers will follow the 186 days Plano ISD school calendar with the exception of new teacher training days.

Tuition
I understand it is my responsibility to obtain, read and follow the financial procedures and business rules outlined in the ECCC Handbook for my child to be enrolled and participate in the ECCC.
I understand the monthly tuition rate per child includes milk and two snacks per day. Parents of infants are expected to provide formula or breast milk and appropriate baby food until the child is able to eat "table food."
I understand tuition for each child is determined according to the age of each child when they enroll. Children are enrolled based on their age on September 1. Children are not promoted during the school year.

Fees
- The $35 Application/ Waitlist Fee is non-refundable and required when completing an application/ waitlist.
- The $85 Registration Fee is non-refundable and payment will be processed at the time the registration form is completed.
- The $200 Annual Supply Fee is non-refundable and required yearly per each child.
- A $30 Late Payment Fee is applied on the 5th of each month
- A $30 late pick-up fee is charged per child picked up 1-10 minutes past 5:30 pm on Monday - Friday. The fee is charged for one late pick up per child. A fee of $1 is charged for each minute per child past the first 10 minutes until the child/ children are picked up. Payment is required within two calendar days of the late pick-up.
- A Holding Fee ("Holding" a spot) is only allowed for expecting mothers of infants up to 5 months old. Tuition will be charged at 50% of the monthly tuition rate for up to three months to hold for an infant spot. If a child is 6 months old or older, the full month's tuition will be charged.
- No Sibling Discount is offered. Tuition is below market rate.

Start Date Tuition
The monthly tuition is due before my child’s start date. When enrolling my child with a start date of 1st-15th of any month, my account will be charged a full month of tuition. Enrolling my child with a start date of 16th - last day of any month, my account will be charged a half month of tuition.
Tuition is due before my child’s start date or my child is subject to removal due to non-payment and a $30 late payment fee charged to my account. I understand the start date I enter online at time of enrollment is the date that calculates the monthly fee. I am fully responsible for the fee charged to my account based on the start date I was assigned by ECCC staff and I selected, regardless if my child actually attends or not.

Non-payment
The ECCCs is a prepaid program and tuition is always due by the first day of the month or the child’s start date, whichever occurs first. After the start date, tuition is always due on the first day of each month. Full payment, including the $30 late payment fee, is required for a child to remain in the ECCCs.
If a child is removed from the program due to non-payment, the parent will be required to re-enroll the child and pay the registration fee again along with any past due balance to attend the Employee Child Care Center. If there is a waitlist at the center for the child’s age group, the parent will be required to pay the waitlist fee again. The contract will be placed on the waitlist in the order it is received and processed accordingly.
I agree to pay the tuition rate when care begins and at the first of each month.

Withdrawal from ECCC
To withdraw my child from the Employee Child Care Center, the account balance must be current and the withdraw/ schedule change must be completed on-line via Eleyo by logging into the Eleyo ECCC Account. I will pay half month’s tuition if the withdrawal date is between the 1st and 15th of any month or pay a full month’s tuition if the withdrawal date is between the16th and the last day of any month. The correct payment amount is required to be paid online prior to completing the withdraw/ schedule change. Please email the Child Care Financial Support Service Office should you have questions about withdrawing.

Method of Contact
I understand the Child Care Financial Support Services Office's primary method of contacting customers is by the email address provided by each customer and it is my responsibility to ensure my contact information is up to date. If I want to be contacted by a method other than email, I must notify the Child Care Financial Support Services Office in writing and provide the contact information.
