Student Absence Request Form
Parent/Guardian, please complete the Absence Request Form and an email or have your student turn in the form to Yvette Jerlow at the Counseling Office. If you would like to email the form, you may send it to Leesa Gibson at email@example.com.
Always make a telephone call and write a note. A parent/guardian must call the Student Safe Line to report a student absence. Call 469-752-5600, option 1 for English, or option 2 for Spanish, next press 3 to leave a message. Leave student name, ID number, the reason for the absence, date of the absence and who is reporting the absence.
A student has 3 days after an absence to turn in a note in order to receive an excused absence. Note should include student's name, ID number, reason, and date of absence. A parent call does not excuse the absence. We must have a note on file.
Leaving School Ill:
If a student becomes ill while at school, he/she should go to the nurse's office. The nurse will contact the parents. The student must then sign out at the attendance office before leaving. The parent will need to send an excuse note within 3 days.
Late Arrivals and Early Releases:
Students must sign in and out at the attendance office. Failure to sign in or out is considered a Truancy Violation
If your student must leave school early, please send a note with them so he/she can be released from class. Upon returning to campus, please bring an Official Doctor's note verifying the appointment.
Non-school activities must be pre-approved by the principal in order for the student to make up missed work. The attendance office has the form that needs to be filled in and turned in before the absence. These absences are considered unexcused with make-up work allowed.
