  • Medication


    It is the child's responsibility to come to the clinic at the necessary time to take medication. Special needs children will be monitored when appropriate. If your child must take medication during school hours, please be sure that the school rules are followed. These rules are necessary for the safety of all children.

    School Rules:

    • Only medication that cannot be scheduled for other than school hours may be given.
    • All prescription medication must be in the appropriate prescription bottle with a prescription label bearing the child's name, the name of the medication, and instructions for giving the medication.
    • Non-prescription medicine must be in the original container labeled with the child's name, the name of the medication, and instructions for giving the medication.
    • If a child requires topical medication during the school day for rashes, itching, etc these medications must be sent from home with a note or a medication permission form completed by the parent. This medication is no longer available in the clinic.
    • All medications must be accompanied by the PISD medical required form completed by the parent. These are available in the school office.
    • All medications must be stored in the school clinic. Only school personnel may administer medication. Children may not keep medicine on their person or anywhere else in the school.
    • Any unused medication will be destroyed. Medication will not be sent home with a student . It may be picked up by a parent at the school clinic.

     

  • Absence, Illness or Injury Note


    A written parental note will be accepted for up to three consecutive days. A physicians written excuse is required for an absence, injury, or non participation which is longer than five days. 

    Note requirements:

    Parents are required to send a note to the classroom teacher when a student returns to school from an absence or injury.
    Parents are required to send a note to the classroom teacher if the student will not participate in recess physical activity.
    A physicians written non- participation note is required for longer than five days of non- participation. A Physicians written note returning the student to physical activity is required before your child may return to physical activity.
    Parents are required to send a note to the P.E. teacher if the student will not participate in Physical Education/Gym class due to illness or injury.
    A physicians written non- participation note is required for longer than five days of non- participation. A Physicians written note returning the student to physical activity is required before your child may return to Physical Education/Gym class physical activity.
