  • PWSH Student Covid Reporting

In an effort to serve our Plano West families better and expedite the COVID reporting and response process, we are pleased to offer a Google form to streamline this task.  This link is to be used only for students that attend Plano West Senior High School.  Other campuses -- not all -- have their own links for this purpose.  Once you have submitted the form, you will receive a response with information on quarantine dates, attendance reporting, and returning to school.  As always, please contact Celene Berry, our school nurse, with any questions.  She can be reached at 469-752-9609 or celene.berry@pisd.edu.  Thank you so much.

Please use the following link to notify your school nurse of student COVID illness:  PWSH Student COVID Reporting
