    To request a certificate of insurance or proof of "self-insurance" coverage, send a written request to the Benefits and Risk Management Department, including a copy of the original document (contract, letter) requesting the information. The written request should provide basic information as to the activity, services, or event to be covered, including the dates and to whom the certificate should be issued.

    Complete this form to report an accident involving a district-owned vehicle. Send this completed form to the Benefits and Risk Management Department.

    Loss Report
    Complete this form and submit it online. It will then transmit to the benefits department for processing.

    Complete this form and submit it to your campus sponsor or coach.

    Chaperone Release Form
    Complete this form and submit it to your campus sponsor or coach.
